As a Hospice RN / RN Case Manager at Heartland Hospice, you will play a crucial role in delivering expert nursing care and emotional support to patients with serious illnesses as well as their families. Serving as the primary point of contact, you will guide patients and families through every step of the hospice journey, ensuring that care plans are tailored to the unique needs of each individual. Your responsibilities will include performing initial and ongoing patient assessments to monitor condition changes and adjusting care plans accordingly. Collaboration with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, and chaplains—is a vital part of your role to ensure comprehensive and compassionate care.
This position requires delivering skilled nursing care in diverse settings such as patients’ private homes, assisted living facilities, or wherever they call home. You will provide hands-on care as outlined in personalized Plans of Care, administer medications, manage symptoms, and educate patients and caregivers on hospice philosophy, pain management, and end-of-life processes. Additionally, your role involves offering emotional and spiritual support to patients and their loved ones, including bereavement guidance and practical training for caregivers to confidently manage the patient’s needs.
Your effective communication of patient updates to the care team is essential to ensure timely interventions and high-quality care delivery. Maintaining accurate clinical documentation and adhering to infection control and compliance standards are mandatory aspects of the position. You will also contribute to quality improvement initiatives and support scheduling needs to guarantee consistent and efficient care.

- Valid RN license or eligibility to obtain
- minimum 1 year of nursing experience in relevant fields
