The Care Transitions Coordinator is responsible for educating healthcare professionals, patients, and families about home care services while simultaneously driving patient admissions and meeting specific admissions and payor-mix goals. The job demands a dynamic professional who can coordinate patient admissions, conduct in-person assessments, document interactions meticulously in CRM systems, and actively participate in community outreach events to promote Amedisys.
