The role of a Home Health Registered Nurse at CenterWell Home Health is central to delivering quality, patient-centered care in patients' homes. The position involves providing admission, case management, and follow-up skilled nursing visits that help patients regain their independence and improve their quality of life. Nurses work collaboratively with physicians and a multidisciplinary team of healthcare professionals, including physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers, to develop, implement, and modify individualized care plans based on patient assessments and physician directives. The nurse oversees patient care, offers hands-on clinical support, and educates patients and their families to optimize outcomes. Supervisory responsibilities include coordinating and overseeing home health aides, certified home health aides, and personal care workers, ensuring proper documentation, and maintaining communication with physicians and clinical managers for continuous care updates.
This full-time position requires a 40-hour workweek and offers competitive pay ranging from $45.00 to $63.00 per visit/unit, with an annual base salary estimate between $70,500 and $96,900, depending on geographic location, experience, education, and certifications. - Diploma, Associate or Bachelor Degree in Nursing
- Minimum one year nursing experience preferred
- Strong med-surg, ICU, ER, or acute experience
- Home Health experience a plus
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
