The role of a Home Health Registered Nurse at CenterWell Home Health is central to delivering quality, patient-centered care in patients' homes. The position involves providing admission, case management, and follow-up skilled nursing visits that help patients regain their independence and improve their quality of life. Nurses work collaboratively with physicians and a multidisciplinary team of healthcare professionals, including physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers, to develop, implement, and modify individualized care plans based on patient assessments and physician directives. The nurse oversees patient care, offers hands-on clinical support, and educates patients and their families to optimize outcomes. • Diploma, Associate or Bachelor Degree in Nursing
• Minimum one year nursing experience preferred
• Strong med-surg, ICU, ER, or acute experience
• Home Health experience a plus
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
