The role of After Hours Hospice Registered Nurse (RN) at Heartland Hospice is a vital position focused on delivering compassionate care and support to hospice patients and their families during evenings, nights, weekends, and holidays. This full-time position requires an individual who can be the calm in the storm—responding promptly and compassionately to patients' urgent needs when regular business hours have ended. The After Hours Hospice RN functions as the primary liaison between patients, families, physicians, and the hospice care team during their shift to ensure that each patient’s physical, emotional, and symptom management needs are met with dignity and grace.
The After Hours Hospice RN is responsible for providing direct patient care and clinical support, collaborating with interdisciplinary teams that include Licensed Practical Nurses (LPNs), Licensed Vocational Nurses (LVNs), Certified Nursing Assistants (CNAs), and other support staff. This role demands excellent clinical judgment and the ability to work independently while maintaining continuity of care in coordination with the daytime hospice team. The nurse will evaluate patient and family needs to guide clinical decisions and implement the prescribed plan of care effectively.
In addition to direct care, documentation is a significant aspect of the role, requiring accuracy and timely completion of clinical visit records and communications. The After Hours RN also plays an active part in quality assessment and performance improvement programs, contributing to the continuous enhancement of hospice care services within the organization.
Ideal candidates bring over one year of nursing experience with a current RN license in the state of employment or eligibility to obtain one. A compassionate heart and the ability to remain calm and dependable in emotionally intense and urgent care situations are essential for success. Experience in hospice, home health, oncology, or palliative care, familiarity with symptom management and triage, and comfort in providing care in home settings or facilities are highly desirable qualifications. Candidates must have a valid driver’s license, automobile insurance, and reliable transportation to perform patient visits during after-hours shifts. Current CPR certification is also required.
