The role of Hospice RN / RN Case Manager at Heartland Hospice is vital in guiding patients and families through their hospice journey, ensuring that every moment counts during this sensitive time. This position requires a dedicated registered nurse who will serve as the primary point of contact for patients and families, delivering skilled nursing care based on individualized care plans. Responsibilities include conducting initial and ongoing assessments to monitor patient conditions, collaborating with interdisciplinary teams to continuously improve care plans, and providing emotional, physical, and spiritual support to both patients and their families. The Hospice RN also educates caregivers with practical training and emotional reassurance, ensuring that families are confident and prepared to care for their loved ones. Working closely with physicians, LPNs, CNAs, social workers, chaplains, and other team members, the RN Case Manager maintains accurate clinical documentation while following infection control and compliance standards. The role demands strong communication skills to report patient updates promptly so appropriate interventions can be taken.   • Active RN license in the state of employment or eligible to obtain
  • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
  • A genuine hospice heart: compassionate, empathetic, and patient-centered
  • Comfortable providing care in diverse settings, including private homes and facilities
  • Strong communication and critical thinking skills in emotionally sensitive situations
  • Valid driver's license, auto insurance, and reliable transportation
  • CPR certification required
