The Hospice Admissions Nurse role at Heartland Hospice is pivotal in welcoming patients and families into the hospice care experience. This position requires a compassionate and experienced Registered Nurse who acts as the face of the organization during admissions, helping patients and families understand their care options during a vulnerable and critical time. Working closely with the Admissions Manager, the nurse will visit patients in various settings such as homes, hospitals, and skilled nursing facilities to assess their eligibility for hospice care. The nurse will provide clear explanations of the hospice philosophy, guide families through the admission process, and coordinate care transitions in collaboration with physicians, case managers, and facility staff. Strong clinical skills are essential for assessing patient needs, documenting findings, and ensuring compliance with regulations. The nurse must also excel in building trusting relationships and offering emotional support during end-of-life decision-making. - Active RN license in the state of employment or eligible to obtain
- 1 plus year of general nursing experience
- valid driver’s license, auto insurance, and reliable transportation
- current CPR certification
- ability to visit patients in hospitals, skilled nursing facilities, or home settings
- meet state-specific licensure and certification requirements
- must be able to complete required documentation accurately and comply with state and federal regulations
- must be a compassionate and empathetic individual committed to patient-centered care
