The Care Navigator role at CenterWell Senior Primary Care is vital in fostering high-quality patient care for high-risk populations. This position requires working within a dynamic interdisciplinary team to identify patients with complex needs and provide targeted interventions to increase access to healthcare and address social determinants of health. Acting as a liaison between patients, care providers, and community resources, the Care Navigator offers education, support, care coordination, and guidance to promote patient self-determination and overall wellness. This role also includes conducting home visits, facilitating transitions of care from emergency rooms and hospitals, and managing behavioral health and social service referrals. Given its mobile nature, the role involves local travel to patients’ homes, clinics, and community-based settings within the Charleston, SC metro area. Work hours are typically Monday through Friday from 8:00 AM to 5:00 PM with occasional overtime as needed.
The ideal candidate is a registered nurse with at least four years of experience in human services, demonstrating a strong clinical acumen and the ability to multitask in a fast-paced environment. Familiarity with Medicaid guidelines, behavioral health conditions, and field safety practices are preferred. Excellent communication and interpersonal skills, compassion, critical thinking, and problem-solving capabilities are essential to succeed in this role. The Care Navigator will have responsibility for managing care plans, supporting patients with chronic conditions, leading interdisciplinary team meetings, conducting psychoeducation groups, and documenting patient encounters in compliance with HIPAA standards. This position offers a competitive salary range between $71,100 and $97,800 annually, along with eligibility for bonus incentive plans and comprehensive benefits. - Minimum of 4 years of experience working in human services and navigating community-based resources

- Ability to work a 40 hour work week Monday through Friday 8 00 AM to 5 00 PM
