The primary role offered is a full-time Hospice Registered Nurse (RN) / RN Case Manager position, providing personalized, meaningful hospice and palliative care to patients and their families. This position operates Monday through Friday, 8 AM to 5 PM, with limited after-hours on-call responsibilities, ensuring work-life balance. The role includes a $5,000 sign-on bonus and competitive annual salary along with benefits such as mileage reimbursement or access to a fleet vehicle. The Hospice RN serves as the vital point of contact for patients and families, delivering skilled nursing care tailored to each individual's Plan of Care. This includes performing initial and ongoing assessments, collaborating with interdisciplinary teams—including physicians, social workers, chaplains, and other nursing staff—to develop and adjust care plans, and providing emotional and spiritual support to patients and loved ones. This role requires maintaining accurate clinical documentation, adhering to infection control protocols, and actively participating in quality improvement initiatives to enhance care consistency and compassion. The RN also educates caregivers on safe, effective care techniques and helps patients and families navigate complex topics such as medication administration, symptom and pain management, hospice philosophy, and the processes associated with end-of-life care.   • Active RN license in the state of employment or eligible to obtain
  • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields
