As a Hospice Chaplain at Gentiva Hospice, you will provide vital non-denominational spiritual and emotional support to patients and their families facing end-of-life care. This role transcends the traditional confines of prayers and presence; it involves offering comfort, guidance, and meaningful connections to those navigating one of life's most challenging transitions. You will be an integral member of an interdisciplinary team, working closely with nurses, social workers, aides, and medical staff to ensure that every patient receives holistic and personalized care aligned with their beliefs and values. The Hospice Chaplain’s responsibilities include assessing the spiritual needs of patients and families, creating individualized spiritual care plans, and conducting visits that may involve prayers, blessings, and rituals tailored to the unique preferences of each family. Additionally, you will assist families through times of grief, loss, and bereavement by providing counseling and emotional support. Building relationships with local faith leaders and community resources will be essential in extending the scope of care and addressing the diverse spiritual backgrounds of patients. Accurate and timely documentation of spiritual assessments, interventions, and outcomes will also be a key part of this role.
