The Community Health Worker (CHW) role at Jordan Valley Community Health Center is critical in bridging gaps between healthcare providers and the community. CHWs play an essential role in facilitating health equity across the Jordan Valley service area by building meaningful relationships with patients and community members who either use the health center's services or are in need of health resources. As liaisons, CHWs connect individuals to vital care and resources, helping to improve patient outcomes and address social determinants of health that impact wellness. In this role, CHWs work closely with partnering school districts to identify health and wellness needs of students and set patient-centered goals. They build and maintain professional relationships with school staff—such as nurses, teachers, and secretaries—as well as community organizations, healthcare providers, and social service agencies. Responsibilities include managing school-based consent forms, scheduling and facilitating various healthcare appointments including dental, vision, well-child, and telehealth exams, and ensuring follow-up on referrals for medical, dental, behavioral, therapy, or vision services. The CHW acts as an advocate for patients and collaborates with their treatment teams through care coordination and case management. CHWs also empower patients and community members by providing culturally appropriate health education and basic health screenings, such as well-child checks, enhancing health literacy and capacity within the community. They support individuals in navigating complex health and social service systems and connect them with resources to meet basic needs. This position demands strong communication skills, cultural sensitivity, and the ability to work effectively both independently and as part of a multidisciplinary team.
