Humana Gold Plus Integrated is currently seeking to add compassionate and skilled Long-Term Services and Support (LTSS) Care Coordinators, also known as Care Coach 1, to their dedicated team. This role is essential in assessing and evaluating members' health and social needs to maintain or enhance their wellness. Care Coordinators at Humana Gold Plus Integrated serve as advocates and guides for members and their families, facilitating access to appropriate resources and coordinating care across multiple service providers. The Care Coach 1 position involves employing a variety of strategies and approaches to manage health issues robustly within the scope of professional standards, regulations, and company guidelines. Although the role operates under established procedures, it requires the individual to exercise judgment in prioritizing work and decision-making. The Care Coach 1 will primarily conduct home visits to Medicaid members, assisted living facilities, long-term care facilities, and other care environments, dedicating 75-90% of their time to local travel. Their core responsibility is ensuring members receive services in the least restrictive setting possible while assessing care needs and planning appropriate interventions. This role requires active coordination and monitoring of case management plans in alignment with the member's personal goals, continuously ensuring progression toward desired outcomes through ongoing assessments and communication. This position demands exceptional interpersonal skills, as the Care Coordinator is the primary point of contact for the member and their families, community partners, and care teams. The role requires a commitment to travel within the region for face-to-face interactions, emphasizing a patient-facing approach. Additionally, this Care Coach role requires compliance with Humana's Tuberculosis (TB) screening and driver safety programs, necessitating valid licensing and vehicle insurance per the applicable state regulations. The role is full-time, scheduled for 40 hours per week, Monday through Friday, with working hours from 8:00 AM to 5:00 PM Central Standard Time, offering schedule flexibility based on business needs. The salary range for this position is estimated between $53,700 and $72,600 per year, reflecting the candidate’s location, experience, skills, and qualifications.
