We are currently seeking a full-time Senior Patient Care RN Manager to join our dedicated hospice team. Reporting directly to the Executive Director or Administrator, this leadership role is critical in maintaining a high-performing hospice program that adheres to stringent quality standards and regulatory compliance. The Senior Hospice RN Patient Care Manager will oversee all patient care operations, ensuring that clinical and administrative workflows are efficiently managed. Responsibilities include supervising nursing and clinical staff, facilitating interdisciplinary group meetings to review and update patient care plans, and ensuring timely completion of patient visits and documentation. This position requires strong clinical knowledge of hospice principles, excellent communication and organizational skills, and proficiency with healthcare software and electronic medical records. The role also involves managing logistical aspects such as referrals, durable medical equipment pickups, Medicare eligibility verification, insurance processes, and patient benefit tracking. As a pivotal leader, the Senior RN Manager will act as a liaison between clinical teams, leadership, and external healthcare providers to uphold the highest standards of hospice care. They will respond compassionately and professionally to inquiries and provide skilled nursing care when needed, including on-call duties. This role demands adherence to hospice regulations including Medicare, Medicaid, JCAHO, and ACHC standards, ensuring compliance in all aspects of patient care management.   • Current registered nurse (RN) license in the state of employment
  • Graduation from an accredited nursing program (Associate degree accepted with relevant experience)
  • Minimum 3 years direct patient care experience
  • Knowledge of hospice principles, regulatory compliance, and managed care
  • Strong communication, collaboration, and organizational skills
  • Proficiency with electronic medical records (EMR), Microsoft Office, and healthcare software
  • Valid driver's license and automobile insurance
  • Ability to provide skilled nursing care including on-call responsibilities
  • CPR certification
