As a Senior Hospice RN Patient Care Manager at Gentiva Hospice, you will play a critical leadership role responsible for maintaining and enhancing the quality and compliance of hospice patient care services. Reporting directly to the Executive Director or Administrator, you will lead and supervise nursing and clinical associates to ensure effective and compassionate care delivery. Your role includes managing clinical and administrative workflows such as referrals, durable medical equipment pickups, Medicare eligibility, insurance verification, and patient benefit tracking. You will coordinate and facilitate Interdisciplinary Group (IDG) meetings to review and update patient care plans and clinical documentation, ensuring adherence to regulatory standards including Medicare, Medicaid, JCAHO, and ACHC. This leadership position demands excellent communication, collaboration, and organizational skills as you serve as a vital liaison between clinical teams, organizational leadership, and external providers. Your responsibilities also encompass scheduling and visit assignments, ensuring timely completion of patient visits, and responding professionally to referrals and clinical inquiries. With a focus on adherence to hospice industry regulations and quality standards, you will foster a high-performing hospice program that meets the needs of patients and their families. Candidates for this role should possess a current Registered Nurse (RN) license, proven clinical expertise in patient care, and an understanding of hospice principles and managed care. This position also requires proficiency with Electronic Medical Records (EMR) and healthcare-related software, a valid driver’s license, and the ability to provide skilled nursing care including on-call responsibilities. CPR certification is mandatory, and while a bachelor’s degree in Nursing and hospice or home care experience are preferred, they are not required.
