The After Hours Hospice RN (Registered Nurse) role at Heartland Hospice offers a unique and fulfilling opportunity for skilled nursing professionals who wish to make a profound difference during critical times. This full-time position involves providing direct patient care and compassionate support outside of normal business hours, including evenings, nights, weekends, and holidays. The RN will serve as the primary liaison between patients, families, physicians, and the broader hospice care team during on-call shifts. Responsibilities include promptly responding to urgent calls, managing symptoms, performing patient visits, and addressing the immediate needs of patients and their loved ones. The RN will play an integral part in ensuring continuous and coordinated care by collaborating with interdisciplinary teams comprising Licensed Practical Nurses (LPNs/LVNs), Certified Nursing Assistants (CNAs), and other healthcare support staff. Documentation of clinical visits and communications must be meticulously maintained to support quality assessment and performance improvement initiatives. This role demands an RN with excellent clinical judgment, the ability to work independently while maintaining continuity with daytime care teams, and a deep-seated hospice heart; compassion, calmness, and dependability are key attributes.
Ideal candidates will possess a current RN license in the state of employment or the eligibility to obtain one, along with one or more years of nursing experience. Additional preferred experience includes hospice, home health, oncology, or palliative care background, familiarity with symptom management and triage, and strong skills with electronic medical records (EMR).
