In addition to transportation, the role encompasses patient advocacy, assistance with Activities of Daily Living (ADLs) such as nutrition, shopping, and minor housekeeping, and working collaboratively with CHR nurses. • Certified or willing to be certified as CHR upon request within one year of employment
• certified in basic life support (CPR) or becomes certified within 3 months of employment
• must have a valid driver’s license and driving record that allows access to CPN vehicles for daily work function
• computer skills required for documentation in the patient record
• must have a high school diploma or GED equivalent
• must have a good driving record and personal automobile insurance
• must have very good communication and written skills
• ability to work a varying schedule as needed
