This position reports directly to the SSN/CHR Coordinator and requires adherence to the established CHR Transport policy, ensuring that all services rendered meet the highest standards of safety and professionalism. The hourly wage range is between $9.00 and $16.00, commensurate with the experience and qualifications brought by the applicant. The Transporter and Assistant is entrusted with vital responsibilities that extend beyond driving; this includes offering supportive assistance to elders with mobility, documentation management, and facilitating a smooth appointment experience.
In addition to transportation, the role encompasses patient advocacy, assistance with Activities of Daily Living (ADLs) such as nutrition, shopping, and minor housekeeping, and working collaboratively with CHR nurses. Candidates will be expected to participate in home visits and community health promotion events, playing an integral role in the continuum of care and community engagement spearheaded by the Senior Support Network. This position demands flexibility, strong communication skills, and the physical capability to assist clients with varying levels of mobility.                 - Certified or willing to be certified as CHR upon request within one year of employment
 - certified in basic life support (CPR) or becomes certified within 3 months of employment
 - must have a valid driver’s license and driving record that allows access to CPN vehicles for daily work function
 - computer skills required for documentation in the patient record
 - must have a high school diploma or GED equivalent
 - must have a good driving record and personal automobile insurance
 - must have very good communication and written skills
 - ability to work a varying schedule as needed
