The Care Coordinator for the Intellectual and Developmental Disabilities (IDD) Pilot Program is a critical role within ILS that focuses on the development and facilitation of comprehensive care plans for individuals with intellectual and developmental disabilities. This position demands a compassionate and organized professional who can effectively manage multiple facets of care coordination to improve health outcomes and promote independence. The Care Coordinator serves as a central liaison, connecting enrollees with healthcare providers, community resources, families, and support networks to ensure that care is seamless and supportive. Key responsibilities of the Care Coordinator include assessing individual needs, identifying care gaps, and crafting personalized support plans tailored to the unique requirements of each enrollee. The role also involves continuous monitoring and adjustment of these plans to reflect changing needs and circumstances, contributing to enhanced quality of life and well-being. The Care Coordinator must maintain detailed and accurate documentation, ensuring compliance with all relevant policies, regulations, and quality standards. This position requires strong communication skills to educate participants and their families about available resources and to foster self-advocacy skills among enrollees. The ideal candidate will bring at least two years of relevant experience working with individuals with intellectual and developmental disabilities, supported by a bachelor’s degree in social work, sociology, psychology, gerontology, nursing, or a related human services field. Registered Nurses and Licensed Practical Nurses with significant experience are also encouraged to apply. Candidates with additional qualifications such as a master’s degree or certification in care coordination or case management will find their expertise highly valued. Experience with Medicaid waiver programs and familiarity with behavioral health interventions will further enhance the candidate’s ability to succeed in this role.   • at least two (2) years of relevant experience working with individuals with intellectual developmental disabilities
  • bachelor’s degree in social work, sociology, psychology, gerontology, or related social services field

  • or Registered Nurse (RN) licensed to practice in Florida
  • or Licensed Practical Nurse (LPN) with a minimum of four (4) years of relevant experience
  • relevant professional human service experience may substitute educational requirement on a year-for-year basis
