The role of a Hospice Registered Nurse (RN) or RN Case Manager at Gentiva Hospice is critical in guiding patients and families through the entire hospice experience. Acting as the primary point of contact, the Hospice RN delivers skilled, hands-on nursing care tailored to each patient’s individual Plan of Care. Care is provided in diverse settings including private homes, assisted living facilities, and other patient residences. Responsibilities include performing initial and ongoing patient assessments, adjusting care plans as needed, and collaborating with an interdisciplinary team consisting of physicians, licensed practical nurses (LPNs), certified nursing assistants (CNAs), social workers, and chaplains.
The Hospice RN also plays a crucial role in providing emotional, physical, and spiritual support to patients and their loved ones. This includes educating caregivers on safe and effective care practices and assisting families through the bereavement process. The RN ensures that clinical documentation is accurate and compliance standards are followed to maintain high-quality care delivery. The position requires strong communication skills, critical thinking, and the ability to handle emotionally sensitive situations with compassion and empathy.
