We are currently seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our interdisciplinary team on a part-time basis. In this advanced practice role, you will work autonomously in collaboration with the Palliative Medical Director to provide exceptional palliative care to patients in their homes. Your responsibilities will include performing comprehensive assessments that cover physical, emotional, and psychosocial aspects, developing and adjusting individualized care plans, prescribing medications including controlled substances, and ordering diagnostic tests pursuant to state law. The role requires managing complex medical needs aimed at improving quality of life while supporting patients and their families through every stage of treatment. This position calls for a practitioner with strong leadership and communication skills, self-motivation, and sound independent decision-making abilities. The ideal candidate will have a solid understanding of palliative medicine principles, hospice and home health care eligibility criteria, Medicare and Medicaid regulations, and the interdisciplinary care process. Experience with Part B billing and proficiency with point-of-care software and technology are important for success in this role. Holding relevant certifications such as ACPHN in Hospice and Palliative Care and maintaining mandatory continuing education requirements are also essential.   • Master of Science in Nursing
  • Current RN license and Nurse Practitioner certification or license as required by state law
  • Minimum two years' Nurse Practitioner experience
  • Collaborative agreement with supervising physician or willingness to establish
  • Part B Provider certification
  • Federal DEA license and state DEA license if required
  • Valid driver's license and current automobile insurance
