The role of Hospice Licensed Practical Nurse (LPN) at Gentiva Hospice is vital in delivering hands-on care and comfort to hospice patients across various settings including private homes, assisted living facilities, and nursing facilities. Reporting directly to the Administrator or Executive Director, the Hospice LPN is responsible for providing skilled nursing care under the supervision of a registered nurse. This care includes administering medications, monitoring patient conditions, reporting changes, participating in infection control, and supporting families through challenging times.
The Hospice LPN is a key member of the interdisciplinary care team, contributing to the assessment of patient and family needs and assisting in the execution of individualized care plans. Empathy and excellent communication skills are essential in this role to support both patients and their families, provide bereavement support, and act as a trusted resource during end-of-life care. Responsibilities may also include assisting with death pronouncements in accordance with state law and under RN supervision, and participating in on-call duties to respond to urgent patient needs.

- Graduate of an accredited Practical Nursing program
- Current LPN/LVN license in the state of employment
- Minimum 2 years of LPN experience in hospice, oncology, long-term care, or home health
- CPR certification required
- Valid driver's license and current automobile insurance
- Ability to work independently and as part of an interdisciplinary team
